The influence of anaesthetic technique on plasma level of IL-6, IL-10 and IL-6/IL-10 ratio during laparoscopic cholecystectomy. TIVA versus inhalation anaesthesia

Abstract

Background. The aim of this pilot study was to compare the effect of two anaesthetic techniques on IL­6/IL­10 ratio as well as on plasma level of IL­6, IL­10 during laparoscopic cholecystectomy.

Patients and methods.
After ethics committee approval, 40 patients ASA I/II undergoing elective laparoscopic cholecys­tectomy were enrolled in the study. Patients were randomly allocated into two groups. Group 1 received TIVA-TCI with propofol-remifentanil and group 2 received isoflurane-remifentanil anaesthesia. Plasma level of IL-6, IL-10 and the ratio IL-6/IL-10 were measured before and after induction of anae­s­thesia and at 2 and 24 hours postoperatively (ELISA, Quanti­kine, immunoassay kit, R & D Systems, Minneapolis, USA).

Results.
Plasma level of IL-6 was significantly higher in group 2 (isoflurane group) as compared to group 1 (TIVA group) at 2 hours postope­ratively (p < 0.05). There were no significant diffe­rences in plasma level of IL-10 perioperatively although the highest difference was observed at two hours postoperatively (p > 0.05). The IL-6/IL-10 ratio was significantly increased in group 2 (isoflurane group) as compared to group 1 (TIVA group) at 2 hours postope­ratively (p < 0.05).

Conclusions.
The results of this pilot study suggest that TIVA suppress the inflamma­tory response to a greater extent than inhalation anaesthesia.
