Drotrecogin alpha-activated in a case of severe sepsis

Abstract

We report the case of a 24yr. old male, admitted with the clinical picture of a septic shock and MDOS due to perineal necrosis, consecutive to a surgical intervention for phimosis. In spite of intensive treatment with antibiotics and the surgical debridement of the necrotic tissues, he presented a sustained hypotension resistant to the use of vasopressors, and respiratory insufficiency requiring mechanical ventilation. After a 4 day infusion of drotrecogin alpha (activated) the clinical picture improved spectacularly. The patient was transferred to the surgical ward on the 8th day after the initiation of drotrecogin alpha (activated) therapy.
